Job Description

Shift: variable. Provides clinical oversight of care delivery, case management, scheduling and staffing. Coordinates and ensures safe, appropriate and timely care to all assigned patients. Continually evaluates staffing competencies. Attends managerial meetings, Monthly case conferences as needed, as well other leadership meetings and/or agency required meetings/trainings. Functions as a resource for all team members in regulatory, educational ,quality compliance related activities. Supports education and quality initiatives.
Current R.N. licensure in the state of Ohio, B.S.N preferred. 3-4 years clinical experience in Home Care or Hospice required. Working knowledge of Home Care/Hospice regulations required
